摘要

本研究運用問卷調查、實地訪查、焦點團體、個別訪談與次級資料分析等方法，檢視我國長期照顧十年計劃架構下，長期照顧管理中心運作與照顧管理實踐情形，並探究照顧管理過程中，制度面、組織面與服務輸送等層面的問題與困境，如長期照顧政策法令與行政體系的零散與分歧、經費不足、照顧管理中心組織定位與功能的侷限，以及需求評量結果的落差、民間照顧資源不足、品質監督管理尚待落實、資訊系統功能未能彰顯等。最後依據研究結果分別從「政策法令與行政體系」、「組織定位與組織功能」、「照顧管理與服務輸送」等層面提出相關討論與建議，諸如明訂長期照顧政策法令、整合長期照顧行政體系、提升照顧管理中心層級、穩定長期照顧經費、強化照顧管理中心人力資源管理、加強宣導以逐步提升服務總量、增進需求評量之一致性與有效性、保障照顧服務員勞動權益、增進民間單位投入意願、落實品質監督管理制度，及建置完善照顧管理資訊系統，以增進照顧管理效率等。

In this study, questionnaire survey method, on-site interview, focus group, individual interview and secondary data analysis methods are used to inspect, under the framework of ”Ten-Year Plan for Long-term Care” of this country, the operation of long-term care management center and the implementation of care management. Meanwhile, the problem and dilemma in the institution, organization and service delivery perspective during the care management process will be investigated, for example, the divergence and difference in long-term care policy regulation and administrative system, insufficient budget, the organization positioning and functional limitation in care management center, the difference in the need evaluation result, insufficient care resource in the private sector, insufficient implementation in quality monitoring and insufficient management information system function, etc. Finally, according to the research results, related discussions and suggestions are proposed from perspectives such as policy, regulation and administrative system, organization positioning and organization function, and care management and service delivery, and they include, for example, the making of long-term care policy and regulation, integration of administrative system for long-term care, the enhancement of the level of care management center, stabilization of long-term care budget, reinforcement of human resource management in the care management center, reinforcement of announcement for the increase of service quantity, enhancement of the consistency and effectiveness of the need assessment, guarantee of the labor right of home care workers, enhancement of the willingness of involvement of the private sector, fulfillment of the quality monitoring and management system, and the setup of better care management information system so as to enhance care management efficiency, etc.
