放射線部檢查報告作業時效之探討

A Study on Quality Improvement of Delivering Examination Reports at the Radiology Department

摘要


患者就醫之等候時問是重要醫療服務品質指標之一。以放射線部各項檢查而言,其報告作業流程所需時問,影響患者在醫院內的等候時間或下次回診時間,其對醫院經營成本與效率亦會產生極大的影響力。本研究主要探討某醫學中心放射線部各項檢查報告作業流程現況、各作業流程問相互關係、及影響作業流程時效的相關因素。於研究設計與方法,本研究採用研究者自行設計之問卷量表,包括作業流程問卷及內部人員問卷二種,分別以門診患者及部門工作人員為研究對象,以了解各檢查報告作業流程時效之影響因素。本研究重要發現,有(l)報告作業流程中,站與站間之時間遲延,是造成檢查報告發出時效長之最主要變異來源;(2)檢查報告作業流程之站與站間,電腦斷層與特殊攝影檢查確實會影響門診普通X光檢查之報告發出時間;(3)普通X光、特殊造影及電腦斷層等三類檢查報告時效,會受到檢查內容、儀器數量及擺設、檢查業務量、工作人員習慣、疾病複雜度、及工作人員請假等因素所影響。

Waiting tint is one of service quality indicators in health care. The length of delivering examination reports not only affects patients' waiting time but their next appointment Also, patients' waiting time usually affect the cost and effectiveness of hospitals in ten of management. The objectives of this study am to investigate the current process of delivering examination reports at the radiology department in one Taiwanese hospital, to see the relationship among different examinations and how they affect each other in of process, and to explore factors which affect the length of delivering the reports. Questionnaires designed by the researcher included two parts in order to address relevant factors influencing the length of delivering the reports. One was section of business process management whose sampling was inpatients; the other one was section of employees' attitudes whose sampling was all departmental workers. The main findings of this study were, first, the main variation of resulting the longer period of time of delivering the reports came from the delay of between stations. Second, CT and special radiography examinations affected the producing of ambulatory X-ray report. Third, the main factors of affecting production of the examination reports were examinations' components, amount and allocation of facilities, work load, employee's behavior, complexity of diseases, and employee's leave.
