技術混合照護模式之實施成效評析-文獻回顧

Reflecting on Intention to Stay of New Nurses from Postmodernism Analysis of the Effects of Skill Mix Model- A Literature Review

摘要


護技術混合照護模式利用非專業的護理輔助人員來協助臨床護理業務,使護理人員專責於專業工作,並可解決護理人員工作負荷及人力不足的問題。本文主要目的是利用文獻回顧方法,來評析技術混合照護模式之實施成效。透過系統性搜尋發表於PsycINFO,PsycARTICLES,PubMed,MEDLINE,CINAHL,AgeLine,Cochrane library及Chinese Electronic Periodical Services(CEPS)資料庫,逐一閱讀全文後共13篇符合本文納入條件。結果顯示護理人員對技術混合照護模式滿意度有逐漸改善的趨勢,在護理照護品質、減緩意外事件及成本效益則依研究方法不同而有不同。以護生擔任護理輔助人員能從事較專業性的護理工作,但須考量溝通技巧及學習態度,未來的研究需再加強研究方法的嚴謹度。實施技術混合照護模式時,除了考量成本效益外,更應關注於護理品質及病人安全的維護。

The skill mix model was applied to nursing assistants in assisted nursing care, in order to increase performance of professional nursing tasks, and to solve nursing loading and shortage problems. The aim of this paper was to use a literature review to analyze the effect of the skill mix model. This study searched databases including: PsycINFO, PsycARTICLES, PubMed, MEDLINE, CINAHL, AgeLine, Cochrane library and Chinese Electronic Periodical Services (CEPS). Thirteen articles were found to meet the inclusion criteria after reading full content. The effect of the skill mix model was that nurses achieved more satisfaction through utilization of the skill mix model. The outcomes for improving quality of nursing care, decreasing accident events, and reducing cost-effect varied depending on different studies. Nursing students as nursing assistants can implement better nursing professional skills compared to general nursing assistants. However, their communication skill and learning motivation should be of concern. Improving the validity of the skill mix model research method is suggested in further studies. Besides considering the cost-effect, nursing quality is also suggested as a skill mix model policy consideration.
