照護一位口腔癌患者皮瓣重建術後之護理經驗
Nursing Experiences of Caring an Oral Cancer Patient Underwent Flap Reconstruction
摘要
本文敘述照護一位第二期口腔癌患者接受口腔皮瓣重建術後，因顏面結構的改變、傷口疼痛及口腔異味，造成身體與心理問題。於2012 年4 月2日至4 月17日護理期間，藉由直按照護、觀察、筆談、會談方式進行資料收集，運用Gordon十一項功能性健康型態評估，確立個案有現存性感染、疼痛、身體心像紊亂三項護理問題。護理過程中藉由完整傷口的評估，提供個案合適的換藥方式，減少傷口惡臭及滲液;透過止痛藥及放鬆技巧減緩疼痛;同時藉由持續性的關懷、傾聽並以同理心鼓勵個案說出內心感受與接納情緒適當發洩；亦藉由護理人員與家屬關心及陪伴以協助重建正向的自我概念，改善身體心像紊亂所帶來的衝擊，使個案能正向面對身體心像改變並引導參與自我照顧，順利適應術後生活。

This case study described nursing experiences of a patient with stage II oral cancer receiving flap reconstruction. The nursing period was from April 2^(nd), 2012 to April 17^(th), 2012. Data were collected by directing care, observation, physical examination, reviewing laboratory data and interviews. Gordon’s 11 Functional Health Patterns was used for a holistic frame to assess and formulate nursing problems and patient’s needs. Wound infection, pain, and body image disturbance were three main nursing problems. Holistic wound assessment, appropriate wound dressing and wound care were provided to solve patient’s wound infection. Meticulous pain assessment and pain relief measures were used to alleviate patient’s pain. Active listening, companion, caring, and psychosocial support were provided to help patient adapt to body image disturbances and return to normal life.
