Dental Management in Patient with Severe Aplastic Anemia - A Case Report

Aplastic anemia (AA) is a rare disease, and often a hematologic disorder in which the peripheral blood cells are decreased because of bone marrow failure. The clinical course often reflects the severity of pancytopenia and is unpredictable for the individual. Hemorrhage and infection remain the major threats to these patients due to the exist agranulocytosis and thrombocytopenia. Therefore dental procedures potentially can cause serious complications and should be applied with caution. Recent advances in transfusion medicine, infection management, bone marrow transplantation, and immunosuppressive therapy have improved survival of patients with AA. A 8-year-old boy with severe aplastic anemia was referred for bone marrow pre-transplantation evaluation. General dental procedure including oral hygiene instruction, pulp therapy and extraction was performed. Combination of modalities such as blood transfusion and dental prophylaxis was also implemented. In this report, the dental and medical management of a patient with severe type AA was presented. The treatment modalities used in this case provide a current concept for patients who are severely medically compromised with AA.

不良性再生貧血是一個少見的血液疾病，主要是由於骨髓造血失敗導致周邊血球減少。臨床症狀的表現主要取決於“全血細胞減少”的嚴重程度，且個體之間的表現也不盡相同。出血和感染對於這類病人來說是兩項最大威脅。近期由於輸血藥物、感染處理、骨髓移植與免疫抑制治療的技術進步，對於這類患者的純活率有顯著提升。本案例為患有嚴重型再生性不良性貧血的八歲男童，照會牙科進行骨髓移植前評估。在適當的輸血與預防性抗生素給予之下，進行的牙科治療包括：口腔衛生教育指導、牙髓治療與拔牙。在這個案例中我們將討論嚴重型再生性不良性貧血的治療。
