運用治療性遊戲提升呼吸道感染病童噴霧治療之成效

Using Play Therapy to Improve Effects of Aerosol Treatment in Children with Upper Respiratory Infection

摘要

呼吸道感染之住院病童，為了濕化痰液利於排出痰液，常規會執行噴霧治療，而噴霧療效果之好壞與按時執行治療及執行噴霧治療方式適當性有極大的關係，筆者服務之兒科病房，常因病童哭鬧不配合，導致未能按時給予噴霧治療藥物，而讓治療大打折扣。本專案希望透過改善方案的實施，來提昇住院病童噴霧治療執行比例。在專案施行期間（2009年7月－11月）由專案小組以查檢表找出影響噴霧治療的主要原因有：病童不合作、治療時間與病童作息衝突。以上問題和護理人員不知如何運用治療性遊戲及欠缺執行噴霧治療的相關知識有關，經由改善措施安排教育訓練及製作遊戲包，增強護理人員運用治療性遊戲之能力及意願。結果顯示教育訓練之課程有助於臨床實務性的提升，滿意度達91.7%；課程內容有助於知識之提升，滿意度達100%。臨床運用治療性遊戲後，病童確實執行噴霧治療的比例從68.6%提昇至94.7%。

This article describes a 5-months project aimed at improving the effectiveness of aerosol therapy in hospitalized children. The project team used a checklist to collect and analyze what influenced the success of aerosol treatment. Results indicated two main factors: uncooperativeness and treatment time disrupts children's sleep. Both factors reflected the nurses' lack of knowledge in proper administration of aerosol treatment in children. Humidified aerosol treatment is often used to clear airway secretion in children with upper respiratory infection. This treatment requires both proper timing and accuracy in the treatment procedures to reach optimal effects in children. However, children usually are to young to cooperate during the treatments. The project team provided nursing training on proper treatment procedures and the use of play therapy in preparing children for treatments. The nurses' post-training evaluation showed 91.7% satisfied and 100% improved knowledge. Adding therapeutic play to better prepare children for the treatment demonstrated an increase of success in the treatments from 68.6% to 94.7%.
