運用資料包絡分析法評估軍醫院營運績效

摘要

醫院的核心價值，在於有效解除病人的病痛，滿足民眾對身體健康的需求，近年來由於全民健康保險醫療給付的種種限制，以及公立醫院面臨自負盈虧的趨勢，使得以軍人為主要服務對象的軍醫院，在經營管理的能力方面，愈來愈受到重視，如何降低經營成本與提高醫療品質，成為軍醫院未來發展的重要課題。
本研究採用資料包絡分析法(data envelopment analysis, DEA)，以經濟學的生產函數定義及線性規劃模式，來評估12家軍醫院的經營效率，透過文獻探討找出適合衡量軍醫院經營效率的投入項與產出項，然後運用DEA的CCR模式與BCC模式，分別分析軍醫院之間的整體效率、技術效率與規模效率值，並探討影響效率的可能原因，進而提出績效改善的項目與目標，以做為各軍醫院經營管理之參考。
研究結果顯示，12家軍醫院的整體效率方面，有5家軍醫院被評估為相對有效率；技術效率方面，則有8家軍醫院被評估為相對有效率；部分軍醫院雖然因規模不同，具有相對極大或極小的投入產出特性，經過敏感度分析顯示，對實證結果無明顯影響，表示DEA非常適合作為評估醫院經營績效的分析工具。另外對於相對無效率的幾家軍醫院，以差額變數分析其參考群集顯示，造成無效率的主要原因為投入資源太多，因此相對效率較差的幾家醫院，應優先考慮降低其投入資源。

Since of the national health insurance begins in Taiwan, the armed forces hospital faced the hard business environment. How to reduce the cost and improve medical quality is the most important question to the armed forces hospital.
This study used the DEA (data envelopment analysis, is a linear programming formulation developed by management scientists, based upon economics principles) to evaluate 12 armed forces hospitals in Taiwan, thru the papers reading to find out the suitable input and output items to evaluate. First, this study applied CCR and BCC of DEA mode to analyze the integrated efficiency, technical efficiency and scale efficiency of each of hospitals. Second, to discover that reasons of inefficiency. Finally, this study wish to find out the items and targets of improve efficiency.
The study result appears: in integrated efficiency of 12 armed forces hospital, there are 5 hospitals is efficient relatively, in technical efficiency of 12 armed force hospitals, there are 8 hospitals is efficient relatively. To analysis the difference variable can find that the inefficiency hospitals were due to input resources too much, so that the inefficiency hospitals want to improve efficiency, they must to reduce the input resources.
