運用高擬真示範教學影片提升呼吸治療師CARESCAPE R860呼吸器操作能力之成效探討

摘要


前言:CARESCAPE R860-GE Healthcare為配備多種量測病人呼吸力學工具的呼吸器,這些新式工具無法使用一般測試肺練習,以致呼吸治療師對於這些技術不熟悉。高擬真模擬教學是一種很好的學習方法,但高擬真模擬器並非每個人都能操作。因此,使用高擬真模擬器拍成教學影片提供學員學習,不失為一種方法。本次研究期望運用高擬真示範教學影片來研究影片自主學習的成效。研究方法設計:本研究使用高擬真模擬器,設計五種臨床教案,並依照教案拍攝成教學影片。使用前測學習問卷、後測滿意度調查和高擬真模擬實作來探討教學影片自主學習的成效。結果發現:在前測學習問卷上,R860使用經驗不熟悉者為87.1%。而在後測滿意度調查,高擬真模擬教學影片能有效協助技術學習,其各項教案比例皆大於85%。另外,接受技術評量的呼吸治療師合計39名,利用操作時間與影片時間的比值來評量影片自學成效,影片自學後技術操作達熟悉程度者合計36人,占全部的92.3%。結論與建議:本研究顯示高擬真模擬影片可幫助呼吸治療師對於呼吸器各項困難或稀少的操作技術的學習。因此,建議未來可推廣此類型的學習模式,除可將寶貴經驗流傳分享,也能使呼吸治療師在臨床學習上更加的快速。

CARESCAPE R860-GE Healthcare is a ventilator equipped with various tools for measuring patient respiratory mechanics. However, because the tools cannot be used in general lung exercise tests, respiratory therapists are generally unfamiliar with these tools. Although high-fidelity simulative demonstration teaching is a favorable method for learners to, not everyone can operate high-fidelity simulators. Thus, using high-fidelity simulative demonstration teaching videos to help learners learn may be viable. Accordingly, this study used high-fidelity simulative demonstration teaching videos to investigate the effectiveness of learners' self-directed learning. Research design: This study utilized high-fidelity simulators, designed five clinical teaching plans, and filmed teaching videos on the basis of the teaching plans. To measure the participants' self-directed learning results after watching the teaching videos, this study used learning questionnaires for the pretest, implemented high-fidelity simulative practical work, and conducted satisfaction surveys for the posttest. Research results: According to the pretest learning questionnaires, 87.1% of the participants were unfamiliar with CARESCAPE R860 ventilator in the beginning. The posttest satisfaction surveys revealed that for all teaching plans, the teaching videos effectively helped more than 85% of the participants in acquiring relevant skills. A total of 39 respiratory therapists agreed to have their skills assessed. The ratio of the practical work duration to video duration was employed to assess the self-directed learning effectiveness. In the end, 36 respiratory therapists (92.3%) were deemed to be "familiar with the technical operation." Conclusion and recommendation: This study showed that the high-fidelity simulative demonstration teaching videos can help respiratory therapists learn how to operate the CARESCAPE R860 ventilator involving difficult and uncommon operating skills. Therefore, it is recommended that this type of learning model can be promoted in the future. In addition to sharing valuable experience, it can also enable respiratory therapists to learn more quickly in clinical practice.
