鼻竇鼻腔惡性血管外皮細胞瘤－病例報告
Malignant Sinonasal Hemangiopericytoma - Case Report
摘要
血管外皮細胞瘤是一種罕見的血管瘤且通常發生於下肢及後腹腔，僅有少數報告發生於鼻腔及鼻竇。臨床上主要症狀為漸進性鼻塞及鼻出血，治療以手術切除為主，由於復發率高，術後須密集且長期追蹤。本院於民國85年3月間經歷1例，患者為74歲男性，主訴數月來有漸進性鼻塞及斷續性鼻出血。理學檢查發現在側鼻腔有一球形腫瘤，經Denker氐手術切除腫瘤，病理報告為惡性血管外皮細胞瘤。術後一週於門診追蹤時發現有殘留腫瘤，因此再接受全部上頜骨切除術及放射治療。不幸於放射治療期間併發敗血症而死亡。由於病例特殊，特提出報告。

Hemangiopericytoma is a rare vascular tumor which is commonly found in the retroperitonium and the lower extremities. Only a few cases of malignant sinonasal hemangiopericytomas have been reported in the literature. The definite diagnosis is obtained by pathologic examination. Surgi-cal removal is the main modality of treat-ment. Because of the high incidence of tumor recurrence, regular and long-term follow-up is necessary. A case of malignant sinonasal heman-giopericytoma with symptoms of progress-sive nasal obstruction and epistaxis is reported. The patient received wide exci-sion via lateral rhinotomy initially. One week later, he underwent maxillectomy for a residual tumor in the maxillary sinus. Unfortunately, he expired due to sepsis during the period of post-operative radio-therapy.
