Nasopalatine Canal as a Landmark in Mesiodens Surgery

使用鼻腭管作為正中贅生牙拔除手術的參考點

摘要

Purpose: This study aimed to use the nasopalatine canal as an anatomical landmark for mesiodens removal surgery; to evaluate the mesiodens- nasopalatine canal distance by using cone beam computed tomography (CBCT); and to estimate a possible correlation between this distance and mesiodens removal operative time (MROT). Materials and methods: The study included 51 mesiodens from 38 patients who were radiographically diagnosed as mesiodens. The shortest distance between the mesiodens and nasopalatine canal was measured on axial-section CBCT. The MROT was recorded. The generalized estimating equation was used to analyze the correlation between the mesiodens-nasopalatine canal distance and MROT. Results: The patients5 mean age was 8.47 years. Contact or absence of contact of the mesiodens with the nasopalatine canal had no statistically significant correlation to the MROT. In the subgroup with mesiodens in contact with the nasopalatine canal, the degree of overlap between the mesiodens and nasopalatine canal significantly correlated to the MROT. In the subgroup with mesiodens not in contact with the nasopalatine canal, the mesiodens- nasopalatine canal distance had no statistically significant correlation to MROT. Conclusion: The MROT is not influenced by whether or not the mesiodens is in contact with the nasopalatine canal. If the mesiodens is in contact with the nasopalatine canal, the greater the overlap between the nasopalatine canal and mesiodens, the longer the operative time needed. If the mesiodens is not in contact with the nasopalatine canal, the operation time is not influenced by the distance between the mesiodens and nasopalatine canal.

目的：本研究使用鼻腭管作爲正中贅生牙拔除手術的參考點，以錐狀射束 電腦斷層掃描測量正中贅生牙與鼻腭管的距離並評估此距離與手術拔除正中贅生材料與方法：本實驗共包含51顆正中贅生牙，來自38位從放射影像診斷出有正中贅生牙的病人。以錐狀射束電腦斷層掃描在軸向平面測量正中贅生牙與鼻腭管的最短距離，並記錄拔除正中贅生牙的時間，再以廣義估計方程式分析正中贅生牙至鼻腭管的距離與拔除正中贅生牙所需時間之間的關聯。結果：病人的平均年齡8.47歲。正中贅生牙和鼻腭管接觸與否與拔除正中贅生牙所需手術時間無明顯相關。當正中贅生牙與鼻腭管有接觸的情況下，正中贅生牙與鼻腭管重疊的程度與拔除正中贅生牙所需手術時間有明顯相關。當正中贅生牙與鼻腭管没有接觸的情況下，正中贅生牙與鼻腭管之間距離與拔除正中贅生牙所需手術時間則没有明顯相關。結論：手術拔除正中贅生牙所需時間受到正中贅生牙是否與鼻腭管接觸影響不大。但是在正中贅生牙與鼻腭管有接觸的情況下，兩者重疊的程度越多所需拔牙時間越長。當正中贅生牙與鼻腭管没有接觸的情況下，拔除正中贅生牙所需手術時間則不受正中贅生牙與鼻腭管之間距離影響。
