運用輔助療法照顧一位肝癌病人的護理經驗

本文為照顧一位B、C型肝炎病毒帶原衍生肝硬化合併肝癌的末期病人，因阻塞性黃疸多次入院之護理經驗，護理期間為2011年3月1日至4月19日。透過觀察、傾聽、會談與身體評估等技巧蒐集病史，以生理、心理、社會、靈性等四層面進行資料整理，發現病人有自我照顧能力缺失、營養少於身體需要、家屬因應能力失調及預期性哀傷等主要健康問題。護理過程中，配合文獻查證，以主動關懷、傾聽、陪伴、同理心及心理支持，運用穴位按壓、音樂療法及懷舊治療與生命回顧等輔助療法緩解病人身體不適，協助病人與家屬互相坦承心中想法，相互道謝、道歉、道愛、道別，達到善終之目的。

This article describes the nursing experience of a patient suffering from hepatitis B virus and hepatitis C virus carrier derived liver cirrhosis and hepatocellular carcinoma at terminal stage due to obstructive jaundice multiple admissions. The period of nursing care was from March 1, 2011 to April 19, 2011. The authors used observation, interview, listening, and physical assessment skills and physical, psychological, social, and spiritual to collate the four levels. It was found that the patient had the following problems: self-care deficit, less body nutrition, anticipatory grief, family members' response to ability disorders and other major health problems. Nursing process, together with literature review, active caring, listening, companionship, empathy, psychological support, the use of acupressure, music therapy, reminiscence therapy, and life review adjunctive therapy were implemented to ease the patient's body and to achieve the purpose of hospice.
