摘要

一個有效且可靠的評鑑系統及工具是政府用來確保機構服務品質的利器，但其必須是根據機構的特性與需求所發展形成的，方可竟功。本研究的主要目的有三點：㈠了解社區型態安養護機構負責人對於政府評鑑項目重要性的看法；㈡探討影響負責人對於評鑑項目看法的相關因素；㈢比較安養護機構負責人與長期照護專家對機構評鑑項目看法的差異性。本研究採橫斷式相關性設計，研究結果顯示93位來自台北市士林及北投地區之社區型態小型安養護機構，負責人的平均年齡為44.68歲（標準差=10.17），多數為已婚(70.97%)且教育程度多屬高中程度(60.22%)。負責人認為在目前的機構評鑑項目中，重要性之前三名依序為安全維護、環境設施以及生活照顧與專業服務；影響其認為評鑑項目重要性的顯著因素有機構床數、機構經營時間及佔床率。本研究建議針對目前安養護機構的評鑑指標，除原有環境中的安全維護與環境設施，還應強化在機構中住民的生理性需求滿足情形，以期提升機構中的服務照護品質。

A valid and reliable accreditation system is essential to assure service quality. Thus, the content and items of an accreditation system must meet the characteristics and needs of service-providing agencies. The purposes of this study were threefold: (1) to understand owners and experts' opinions regarding critical accreditation items; (2) to understand factors correlated to owners' opinions regarding critical accreditation items; and (3) to compare the differences between owners and experts' opinions regarding critical accreditation items. This study used a cross-sectional design with a descriptive correlation approach. A total of 93 owners were from Shihlin and Peitou District of Taipei and 7 experts participated in the study. The mean age of owners in the study was 44.68 (SD=10.17) years. Most (70.97%) were married, and 60.22% held high school degrees. Safety maintenance, environmental facilities, and quality of life care and professional service ranked, in order, as the most important accreditation items. Significant factors identified as influencing owners' opinions included duration of facility operations; number of beds in a facility; and occupancy rate. Based on the results of this study, a proposed approach by which to improve the current accreditation system used by community-based long-term care (LTC) facilities is to emphasize the assessment of residents' physical needs and environmental safety as they relate to service care quality.
