The demand of hospice palliative care is growing because population in Taiwan is aging. This review article illustrates the international perspective of hospice palliative care development and challenges they faced. Value experiences in the America or Europe countries which have longer history and integration are summarized for government and related professionals to consider. The conclusions are as follows. Patients received hospice palliative care are not only suffer from cancer but also varies diseases, especially in the elderly. The development and payment policy of hospice palliative care should be coordinated with long term care system. Moreover, caregiver and bereavement support is necessary and should be offered systematically. Well trained volunteers will be an important human resource. In addition, the death education should be enhanced. In the end, how to face the challenges of hospice palliative care should be considered as early as possible.
