摘要

本篇報告是一位受家庭暴力婦女於急診室求醫的照護經驗。個案由先生陪同就診，先生表示是個案自己不小心砍傷的，事後女兒到醫院向醫護人員表示是父親用刀砍傷母親，急診醫療團隊立即給予保護隔離的措施，並以家庭暴力作業流程處理，檢傷分類一級，個案以「8585」代稱，以避免二度傷害，並藉由會談、傾聽、觀察收集相關資料，發現個案主要健康問題為：恐懼、個人因應能力失調、無望感。在急診就診期間，除給予醫療處置、驗傷、通報、診斷書開立外，更提供安全的環境，以維護個人隱私、增加安全感，給予關懷護理，應用傾聽、陪伴、同理心，提供家庭暴力相關知識及社會支援，並建立信任的治療性關係，協助個案接受自己，增加自信心，勇敢面對問題，走出家暴的陰影。

This article describes a nursing experience in the emergency room for a woman suffering from domestic violence. The patient's husb and accompanied her to go to a doctor and described how the patient had cut and injured herself because of carelessness. However, the daughter went to the hospital afterwards to inform medical personnel that is was father who cut and injured her mother with a knife. The medical group of emergency care offered immediate protection and isolation measures. This was followed by emergency documentation as part of the operative procedure of domestic violence, including examination and classification of the first-class wound. The patient name was substituted for number ”8585” to avoid two degrees of injury.
The methods of data collection used were included face-to-face interviews, listening with conviction, and observation. The major health problems of the case included fairness, proper care in conformity with ability, and hopelessness. Through the medical and nursing interventions, we offered a safe environment to ensure the personal privacy and a sense of security. We also gave attentive nursing care and used skills of listening with conviction, accompanying, empathy, providing relevant knowledge of domestic violence, and establishing health education to help the patient build a physician-patient relationship and accept herself, enhance confidence in herself, face the problem, and avoid domestic violence.
