摘要

身體約束於精神科護理是重要的護理措施之一，若執行不當可能傷害病患與工作人員的安全。本專案的目的在改善精神科病房護理人員對身體約束用物及其操作能力；以訪談、自擬問卷調查、分析後，確立問題為：護理人員對身體約束認知不足、約束用物操作技術不佳且操作技術缺乏一致性、操作約束用物無查核以及身體約束用物材質與設計不良等。專案實施後護理人員身體約束認知一致性由67%提升到88.4%。身體約束用物操作技術一致性由30%提升到95%。對身體約束用器材的滿意度得分由24分提升為36.6分。人為損害事件由9件下降為1件。研究結果可作為同質性病房或類似機構的護理人員改善身體約束處置安全性之措施的參考。

Physical restraint is one of the most important nursing measures, and may endanger patients and staff if not used properly. The project aimed to improve the knowledge and skills of the use of physical restraints. After interview and survey with a self-designed questionnaire, the main issues related to the misuse of physical restraints were insufficient knowledge about the use of physical restraints, inadequate skill and consistency in the use of physical restraints, no regular check for the skill of using equipment for physical restraints and poor design and material of physical restraint equipment.
After execution of the project, the consistency of the knowledge about the use of physical restraints was raised from 67 percent to 88.4 percent, consistency of the technique of use of physical restraint was raised from 30percent to 95percent, the satisfaction of physical restraint equipment was raised from 24 points to 36.6 points and artificial damage was reduced from 9 incidents to 1 incident. The results could serve as a reference for the improvement of the safe use of physical restraints for nursing staff in similar wards or comparable facilities.
