加護病房護理人員死亡處理能力之探討

摘要

目的：探討不同科別加護病房護理人員之死亡處理能力提出建議。方法：採用Bugen's死亡處理能力量表探討不同科別加護病房護理人員之死亡處理能力，以變異數分析和階層迴歸法進行統計檢測。結果：死亡處理能力以「死亡思考表達」得分較高，其次為「生命省察」、「瀕死處理」，得分最低為「處理失落」。不同科別加護病房護理人員的省察生命有顯著影響；重症安寧照護經驗和接受臨終照護課程者其瀕死處理、死亡思考表達有顯著影響。結論：瞭解加護病房護理人員在面對死亡處理過程的需要及思考死亡相關的事實等能力，引導其省思在重症臨終照護過程中面臨的困境，並藉由臨終照護課程深化以培養護理人員面對及處理死亡的能力。

Purpose: To explore death competence in nurses with different specialties and provide suggestions. Method: Employing Bugen's Coping with Death Scale to examine death competence among nurses with different specialties, and analyzing the data through ANOVA and hierarchical regression. Result: ＂Thought and Expression about Death＂ scored highest on the death competence scale, and the scores for ＂Reflecting on Life＂ and ＂End-of-Life Care＂ placed second. The lowest score was ＂Handling Loss.＂ There was a noticeable impact on reflecting on life in ICU nurses, depending on specialty. There was perceptible change to End-of-Life Care and Thought and Expression about Death in nurses with palliative care experience for critical care patients, and nurses who had taken courses on end-of-life care. Conclusion: By understanding the abilities that ICU nurses possess regarding the need to face the process of handling death and thinking about truths related to death, this study can provide them guidance in their reflection on the difficulties that arise from the palliative care process for critical care patients. The study can furthermore, through the deepening of hospice care courses, strengthen the competence of ICU nurses in facing and handling death.
