The Taiwanese population is aging rapidly, placing much impact on the government's long-term policies of gerontology; rolling policy revisions have also tested the resilience of both central and local governments. Cijin Island is an urban district of Kaohsiung City that has inconvenient external transportation and poor resources, but is not recognized as an outlying island. Middle-aged people leave the island to work, while the elderly live alone or elderly couples care for each other. The Kaohsiung Municipal Cijin Hospital used strategies of "cohesion local, arousing consensus", "insight trends, deploy early", "courage to expand, active marketing", "empowered community, sustainable development" and built an Integrated Community Service Center in Cijin within four years, including thirteen composite services with a 115-bed long-term accommodation center and five Grocery Store Care Services. We record and share our experience with other participants in long-term care for reciprocal learning.
