精神衛生護理師培訓內容-建構行爲治療實務能力

Constructing the Clinical Competence of Behavior Therapy for Psychiatric Mental Health Nurses

摘要


本研究目的在建立精神衛生護理師行爲治療之臨床實務能力培訓與甄審重點。運用行動研究法,以精神衛生護理學業界專家爲研究核心小組進行文獻查證與討論,並邀請資深精神衛生護理人員爲種子成員參與能力培訓研習會,再由種子人員依據行爲治療實務能力項目進行臨床觀察與試驗,反覆討論、驗證並修正。形成研究結果確認行爲治療實務能力必備要項:(一)與個案建立良好人際關係、(二)評估個案行爲問題、(三)評估個案改變意願、(四)評估個案學習能力、(五)確立個案問題行爲、(六)擬訂行爲治療計畫:目標行爲、治療期間、應用改變行為理論與技巧、行爲觀察與記錄方法、(七)獲得醫療團隊之支持與配合、(八)向家屬及病志說明治療目標、方法、過程、角色與責任、可能結果、(九)獲其同意簽署行爲治療契約書、(十)定期評估與修正、(十一)治療結束總評值。除以上能力項目外,撰寫行爲治療個案實例為能力培訓與甄審之鈴要文件,此實例需涵蓋個案簡介、問題行爲分析、問題行爲確立、目標行爲基礎線創量、行爲治療計畫、行爲治療實施步驟、行爲治療結果等重點。以上能力項目及內容應併入精神衛生護理人員分經進階教育訓練計畫,並作爲行爲治療臨床實務能力之甄審依據。

The purpose of this study was to establish clinical competencies in behavior therapy for psychiatric mental health nurses. Commissioned intervention action research was used in the study. A group of experts were engaged to develop and implement competencies for behavior therapy in clinical practice. These psychiatric mental health nursing experts and scholars in the core committee established the content and guidelines of clinical competence in behavior therapy. The competencies were ability to develop a therapeutic relationship with the client, ability to assess the client’s problem behaviors, motivation for change and learning potential, ability to identify the client's problem behavior , ability to plan the behavior therapy protocol, ability to explain the treatment goal to client and families, understanding of the treatment methods and processes, understanding of the roles and responsibilities of client and therapist, understanding of the probable results, ability to reach agreement with patient about behavior therapy, ability to periodically assess and modify the treatment plan, ability to evaluate treatment effects. The content of clinical case reports includes case introduction, problem behavior analysis, baseline date of problem behavior, behavior treatment plan, steps of treatment, effects of therapy. The above content may be integrated into the clinical training program, to construct a systematic and staged training process for psychiatric mental health nurses.
