以液相層析串聯質譜儀進行毛髮中檳榔鹼與菸鹼及其代謝物之分析

摘要


在台灣地區,口腔癌病患有近九成以上有嚼食檳榔的習慣,其中九成的檳榔嚼食者有吸煙的習慣。儘管嚼食檳榔與吸菸並不違法,但這兩種嗜好卻會帶給身體相當大的危害,兩者合併使用,罹患口腔癌之危險率,較一般人或只有其中一項不良嗜好者高出數十至百倍,為了自身的健康以及避免為家庭與社會帶來負擔,早日戒除應是刻不容緩的。 本研究開發了精確、快速且靈敏的毛髮檢測方法,同時偵測檳榔與香菸之生物指標:檳榔鹼、菸鹼與其代謝物共五種化合物,期待能用於吸菸和嚼檳榔之戒治檢驗上。研究利用簡單的浸泡前處理,吹乾、回溶後,直接進行液相層析串聯質譜儀 (LC-MS/MS) 分析。使用20 mg頭髮,最低檢測極限 (LOD) 可至 5 pg/mg;最低定量極限 (LOQ) 均在 10 pg/mg以下;在濃度 10 ~ 1000 pg/mg 範圍內具有良好之線性關係,線性相關係數 (r2) 均在0.995 以上;同日異日精密度與準確度 (CV%) 皆小於 10%。最後成功的應用於真實毛髮檢體之定量分析。

In Taiwan, over 90% of oral cancer patients have a habit of chewing betel nuts and 90% of them smoke as well. Even though it is not illegal to chew betel nuts and smoke, these two habits do seriously jeopardize people’s health. For people with these two habits, the probability to suffer from oral cancer is ten to hundred times higher than others. To protect ourselves and prevent being burdens for the family and the society, quitting chewing betel nuts and smoking must be done soon. In this study, a specific, rapid and sensitive liquid chromatography tandem mass spectrometry (LC-MS/MS) method was developed to measure the biomarkers of betel nuts and cigarettes, which has five chemical compounds of areca alkaloids, tobacco alkaloids, and their metabolites. Hair sample (20 mg) was washed, weighted and incubated overnight, then evaporated and reconstituted for LC-MS/MS analysis. The method showed a good linearity (r2 > 0.995), precision and accuracy (< 10%) over the concentration ranges of 10 - 1000 pg/mg. Consequently, it was successfully applied for the rapid quantification areca and tobacco alkaloids in human hair for clinical laboratory.
