Cenerally, the best treatment for patients with skeletal Class l malocclusion with anterior crowding or protrusion is extraction of four first premolars. But, when missing tooth of one quadrant is noted, preservation of the first premolar in the same quadrant is an alternative choice. Herein, we reported a case with upper anterior protrusion and missing of upper left central and lateral incisors due to a traffic accident. In considering tooth preservation for future prosthetic reconstruction, asymmetric extraction treatment was planned. After orthodontic treatment and bridge reconstruction, the esthetics of anterior teeth and occlusion of full mouth were substantially improved.
