俯臥自行道氣用於一位嚴重急性呼吸道症候群:病例報告

摘要


俯臥通氣可以改善氧合情形,常被應用於輔助機械通氣,但很少被用於自行呼吸病患,本病例報告一位嚴重急性呼吸道症群病患俯臥自行通氣經驗,俯臥讓符合呼吸衰竭面臨插管危機患者迅速改善缺氧并避免插管,這雖然有待進一步研究,但如果俯臥自行通氣能讓一些嚴重性呼吸道症候群病患避免插管危機,將有助於照顧此類病患。

Prone position ventilation has been known to improve oxygenation. It has often been used as an adjunct to mechanical ventilation but seldom suggested in patients able to breathe spontaneously. We present our experience with the prone position in a spontaneously breathing patient with severe acute respiratory syndrome (SARS). Although further clinical investigation is required, prone positioning allowed us to postpone endotracheal intubation in this patient with impending respiratory failure and rapidly reversed her severe hypoxemia. If the prone position could help prevent intubation, it would be important information for treating patients with SARS.
