末期膀胱癌護理:一個案報告

Advanced-Stage Bladder Cancer Nursing: A Case Report

摘要


近年來台灣積極推廣『安寧療護』的理念,希望提供癌症末期病患身、心、社會及靈性各層面的照護,以提升末期患者生活品質。本文是探討一位膀胱癌患者在經歷疾病疼痛、無望感、家庭因應能力缺失過程之護理經驗。在91年4月2日至4月26日的護理期間利用參與式之觀察方法收集個案語言或非語言行為,以及Gordon十一項功能型健康型態評估為依據,確立其護理問題為疼痛、潛在危險性傷害/跌倒、營養狀況改變、無望感、家庭因應能力等四項。筆者針對各層次護理問題,以關懷、陪伴、傾聽、精油按摩給予提供個別性適當的護理措施,支持個案及家屬一同渡過癌症末期的各種身、心、靈及家庭因應能力問題,使其能在病情穩定出院。期望藉此護理經驗提供給日後照護類似病患之些微參考。

The idea of hospice palliative care in patients with terminal disease has actively been proposed recently in Taiwan in the hope to provide total care including physiologic, psychological, social and spiritual aspects and to improve the quality of life. We herein report our nursing experience in a patient with bladder cancer who suffered from disease-associated pain, hopelessness, and deficient family adaptation .During the period of nursing attended in 2nd~26th April 2005,we collect and analyzed this patient's either linguistic or non-linguistic behavior based on the 11-item Gordon functional health style sheet. His main nursing problems included pain, potential dangerous injury/falling, altered nutritional status, hopelessness, and deficient family adaptation. We had appropriately provided tailored nursing management in the forms of concerning, accompanying, listening, and aromatherapy message in order to assist the patient and his family to overcome their sufferings and discharge uneventfully. This unique experience may help other colleagues when caring similar patients.
