The Quick Neurological Screening Test: Psychometric Considerations

職能治療師常需評估及治療具有學習障礙問題或傾向的兒童,爲早期篩檢出日後可能形成學習障礙的兒童,幷提供此類兒童預防性治療,學校系統及早期療育等機構之職能治療師亟需具備信度與效度之篩檢工具。「簡明知覺-動作測驗」涵蓋神經學、神經心理學與發展學之檢查,可供早期發現與學習障礙及神經失調相關之行為。本文旨在回顧幷評析此測驗之心理計量研究,以供職能治療師選擇適合的發展評量工具時之參考。回顧結果顯示該測驗具備可接受之施測者間信度與施測-再施測信度。鑑別效度方面之研究則顯示,具學習障礙之美國兒童在該測驗上之表現顯著地比無學習障礙之兒童差。未來研究應針對不同文化背景之兒童進行鑑別效度之再驗證。此外,可能影響測驗表現之相關因子(如年齡、性別、慣用手、施測情境等)也值得探討。這些研究將有助於進一步暸解該測驗之心理計量特性,並將對該測驗之發展改良有所助益。

There is a crucial need for valid and reliable screening instruments in occupational therapy practice in order to identify children with or at risk for learning disorders (LD) early and to provide them with opportunities for timely interventions. The Quick Neurological Screening Test (QNST) contains aspects of neurological, neuropsychological and developmental examinations. This test represents an early screening tool that focuses on behavioral parameters associated with learning disorders and neurological dysfunction. This article reviews the research relevant to the psychometric properties of the QNST in order to assist occupational therapists in deciding whether and how this test might meet his or her needs in clinical practice. The review reveals that the QNST has possessed satisfactory interrater and test-retest reliability and successfully discriminated American children with and without LD. Recommendations for instrument validation in relation to the QNST are discussed.
