術前焦慮會影響病人術後恢復、疼痛程度等與手術時心理準備狀態,身為恢復室護理人員在扮演術後照顧是主要角色。本研究藉由「手術麻醉恢復過程與照顧」護理指導光碟作為介入措施,比較與一般術前護理指導單張對緩解首次採腰椎麻醉手術病人焦慮狀況之成效。研究設計為類實驗研究,採立意取樣,收案對象為首次採腰椎麻醉接受手術病人,共收對照組62 人與實驗組60 人。兩組在手術前填寫「半身麻醉認知量表」、「情境與特質焦慮量表」及「貝氏焦慮量表」等量表。實驗組接受「手術麻醉恢復過程與照顧多媒體護理指導光碟」介入措施,對照組則提供常規護理指導(書面護理指導單張),在手術日及術後第一天,再進行二次後測。資料以SPSS 19.0 套裝軟體進行統計分析,研究結果發現兩組在前測時無差異存在(p > .05),但在後測時實驗組認知程度明顯提升並達統計顯著差異(p < .001);在焦慮程度則實驗組明顯降低,達到統計顯著差異(p < .001),顯示提供多媒體護理指導可以有效提升對麻醉的認知並獲得正確資訊,降低焦慮感受,發揮手術全期之護理專業,提升優質的護病關係。
Preoperative anxiety affects both a patient's psychological status and the degree of pain experienced by him or her during postoperative recovery. A recovery room nurse plays a vital role in a patient's postoperative care. The purposes of this study were as follows: (1) to investigate "anesthesia recovery and care" health education intervention impaired by means of an educational DVD designed by recovery room nursing staff, and (2) to compare the effectiveness of alleviating anxiety in patients undergoing spinal anesthesia for the first time who have only previously been educated about the general surgical routine. A quasiexperimental design was adopted for the participants in this study, purposely sampled from patients in the surgical wards of a medical center in Taipei. Participants were referred to as those who had undergone surgery following spinal anesthesia and assigned to either a control group (routine care, n = 62) or an experimental group (multimedia health education, n = 60). The structured questionnaires included the State- Trait Anxiety Inventory and the Beck Anxiety Inventory questionnaire and tested perceptions of spinal anesthesia. The questionnaires were administered to the participants to gather pretest data. Posttest data were gathered after intervention (either routine health education〔control〕or the multimedia educational DVD〔experimental〕). Data collected was statistically analyzed using SPSS 19.0 software. The results indicated that knowledge of spinal anesthesia in the experimental group increased significantly after viewing the multimedia educational DVD ( p < .001). In addition, anxiety scores in the experimental group decreased significantly compared with those in the control group ( p < .001). Educational multimedia offers care and guidance and effectively raises awareness regarding anesthesia among patients as well as provides suitable medical information, thereby reducing patients' anxiety. Through this method, nursing staff can perform their professional duties during the pre-, mid-, and postoperational period while simultaneously improving the quality of their work.
