Cutaneous Metastasis from Hepatocelular Carcinoma: Report of a Case

肝細胞癌併表皮轉移:一病例報告

摘要


肝細胞癌常轉移至肺、腎上腺、骨頭與淋巴結,而表皮轉移極爲罕見。我們報告一位69歲男性肝細胞癌病患,發生表皮轉移。像本例肝細胞癌切除後,以表皮爲復發之主要部位,據我們所知以往文獻未曾提及。回溯文獻,肝細胞癌表皮轉移之常見表徵,似化膿性肉芽腫,毛細管擴張肉芽腫,或藍色小結表皮呈些微紅色。本案例預後良好,不同於其他案例。主要原因在於開刀後並未有肝臟內腫瘤復發的情形。

Hepatocellular carcinoma (HCC) usually metastasizes to lung, adrenal gland, bone, and lymph nodes, while cutaneous metastasis from HCC is a rare event. Herein, we report the clinical details of a patient with cutaneous metastasis from HCC. To the best of our knowledge, this is the first case reported in the literature demonstrating that after surgical resection of HCC, isolated cutaneous metastasis becomes the first manifestation of cancer relapse, while no intra-hepatic recurrence is present. As described in other previous case reports of HCC cutaneous metastasis, the general appearance of such lesions resembles pyogenic granulomas, granuloma teleangiectaticum, or reddish-blue nodules. However, different from those described in the literature, a good prognosis was observed in this patient owing to the absence of intra-hepatic recurrence.
