摘要

本文報告兩個作全厚度角膜移植之成功病例，病例一曾罹患綠膿桿菌角膜炎，病例二為外傷性角膜白斑。在手術過程中若在接受者的角膜實質注入空氣，小氣泡會伸入實質層間但不侵入Descemet氏層，因此可使Descemet氏層與實質層間之剝離較易，手術困難度減輕。上述兩病例並無特殊併發症，接受者與捐贈者之介間並無變白，影響視力之現象，並不如Morrison及Swan在1985年所說，2病例追踪一年視力結果為0.8及0.9。其中病例二在手術過程中曾發生角膜微穿孔，在手術後於前房注入Healon，結果仍令人滿意。此手術法最大的優點為可採用內皮細胞功能不良或甘油貯在之角膜捐贈者，而排斥的發生率甚低，但需要較純熟的手術技巧。

Two successful cases of full thickness lamellar Keratoplasty to treat leucoma cornea were reported. Intrastromal injection of air will facilitate to identify the Descemet's membrane during dissection of recipient stroma. There was no serious complication in these two cases. And, the graft-host interface was clear after one year follow-up. Although microperforation of Descemet's membrane in case 2 resulted in formation of cleft between graft-host interface postoperatively, intracameral injection of Healon made the interface reattached well. The final visual outcome was satisfactory.
