台灣地區核醫實驗室技術人員工作壓力源及其因應之研究

Survey on Pressure and Pressure-regulation of the Nuclear Medicine Technicians in Taiwan

摘要


目的:本研究主要目的爲瞭解核醫科技術人員壓力源的種類、壓力之調適行爲、舒解壓力方法及對象進行研究調查,研究結果希望能提供單位主管及醫院行政主管更加重視技術人員的心理問題之發展,以及作爲壓力源輔導之依據。 方法:本研究針對台灣地區區域級(含)醫院以上的核子醫學實驗室,包括醫學中心(6家)及區域醫院(19家)的核醫科技術人員(包括醫檢師與放射師),以醫院爲單位實施結構不記名問卷調查,主要目的爲瞭解核醫科人員壓力源的種類、壓力之調適行爲、以及改善壓力之措施。 結果:經分析結果,在工作壓力源調查部份,同仁主要壓力源來自於其本身之工作狀況(17.9%),其次爲自我要求(17.8%)、和人際關係(15.9%)。而實驗室認證相關作業也成爲技術同仁所面臨的主要壓力來源之一(15.9%)。在技術人員的調適壓力行爲調查分析中則以努力去克服突破難關(6.4%)所佔的比例最高,其次以面對壓力接受事實及尋求解除壓力方法(6.2%)。在工作遭遇壓力時以向工作同仁(14.9%)及朋友(14.9%)傾訴爲最多,其次爲和家人討論(13.8%),和過去有此類似經驗的前輩討論(11.6%)。 結論:本研究在壓力源分析研究顯示,核醫科技術人員,認爲工作狀況爲最大的壓力源所在,而工作狀況的壓力源,主要以主管所規範的工作事項無法完成或遵守爲最大壓力源所在,因此單位主管和行政管理部門應重視技術人員之人力評估,應包括技術人員的合理工作量、人力運用的準則、工作安排的合理化等,此壓力源對醫療品質的影響,值得再分析與研究。

Background: Nuclear medicine studies, either in vivo imaging studies or in vitro radioimmunoassay, are important in clinical approaching for a precise or early diagnosis. Technicians play the most essential role in this issue. Working incensement, quality improvement, and self acquired etc. have them under the pressure of over loading. Therefore, safety precautions and preventing radiation exposure must be considered by hospitals. Methods: We used a structural anonymous questionnaire given to the medical technicians of regional teaching hospitals and above in Taiwan area. A total of 195 questionnaires were sent to 25 hospitals and the return rate was 61.5% (120/195). Three major stress issues were discussed, including the source of pressure, the self-regulation of pressure, and the attitude to face with the pressure. Each issue was then divided to several related problems. Results: In terms of the source of pressure, the major pressure comes from the work loading (17.9%), followed with self acquire (17.8%), and the staff interactions (15.9%). Laboratory accreditation will be the trend of laboratory development, and this also become a pressure source that bothers the staffs (15.9%). In terms of self-regulation to the pressure of technicians, most of the staff will go by more effort to prove it (6.4%), others may face the pressure and accept it (6.2%). The analysis of the staff's attitude showed that most of the staffs will talk with co-workers (14.9%) or friends (14.9%), followed by their families (13.8%), and then with who had the same experience (11.6%). Conclusion: Pressure per se could either improve or disrupt working performance. We found that, only limited ratio of staffs will tell and discuss it with their departmental directors. This might be reflected that some problems were not thoughtful to the management level and some problem might be under-estimated by the coordinators. The security of nuclear medicine laboratories requires diligent management, including annual plans for staff re-education and periodic spot testing, and a practical system of rewards and punishment in order to promote the quality of laboratory security.
