提昇勞工健康檢查指定醫療機構品質之可行作法研究

勞工健康檢查是職業衛生預防與保健重要措施之一。勞工健康檢查制度未能落實，以致未能達到理想的勞工健康保護之實質意義。本研究為提昇健檢指定醫療機構健檢品質，透過文獻收集、與指定醫療機構自我評估調查、及實地訪視之方式，探討相關問題並提出具體且可行作法，提供政府部門制定與推動健檢政策及事業單位執行健檢業務之參考。本研究發現：(1)巡迴健檢量較到院健檢量為多，(2)健檢指定醫療機構有健檢外包情形，且未建立稽核管理制度，以致外包檢驗室巡迴健檢採集回來之檢體未依標凖作業流程處理與分析，影響檢驗之品質；(3)巡迴健檢之聽力測定，未依標凖聽力檢查之程序，嚴重影響聽力檢查之品質。本研究建議事業單位應建立完整暴露史及工作史，主動提供健檢誓師健康管理評估參考，並仿效國外提供「健檢日」，鼓勵勞工至醫院健檢為優先考量，並要求健檢醫院提供聽力檢查室之背景噪音測定紀錄及儀器校驗報告等資料，及巡迴健檢檢體採集、運送、及保存之標準作業流程資料，共同確保健檢品質。

A labor health examination is one of the important measures for occupation hygiene prevention and health protection. If a labor health examination system cannot put into effect, a perfection of the essence value of labor health protection cannot be reached. In order to improve quality of health examination for medical institution which is assigned for health examination, this study through the literature collection, investigate the assigned medical institutions' self-evaluation, and visit on the spot to confer related problem and provide a specific and practicable way as a reference for government to establish and get health examination policy moving. This study also can be a reference for enterprises which want to implement the business of health examination. This study found out: (1) The examination volume of a circuit health examination is more than the volume of health examination in hospital. (2) The medical institution which is assigned for health examination reassigns the examination to other enterprises and did not establish inspection management system. Because of enterprises did not follow standard operation procedures to handle and analysis of health examination sample, the quality of health examination can be affected. (3) The circuit health examination did not follow hearing examination procedure to do hearing test which is seriously affect the quality of hearing examination. This study recommends enterprises to establish integral exposure history and lob history as a reference to provide health examination doctors for health management evaluation. And follow the example of other countries to provide health examination date to encourage labor to have their health examination in hospital as their priority choice. Moreover, health examination hospital should be asked to provide record of background noise test of hearing examination room and instrument calibration report, and the standard operation procedures for the collection, transfer, and preservation of circuit health examination sample etc. in order to assure health examination quality.
