皮下結締組織移植於牙根覆蓋手術之應用－文獻回顧

摘要

皮下結締組織移植(subepithelial connective tissue graft)於牙根覆蓋手術(root coverage procedure)已有二十餘年之歷史。但對皮下結締組織移植於牙根覆蓋術之效果，尚未有一整合性之文獻回顧。本篇回顧討論了1.皮下結締組織移植手術對於牙根覆蓋手術之臨床效果，及以病人觀點為中心的效果。2.擷取皮下結締組織移植之手術方式3.皮下結締組織移植手術之組織結構4.整合性分析。結果發現皮下結締組織移植對於牙根覆蓋手術可以提供良好的效果，並能減少牙齦萎縮，增加角化牙齦及牙周附連；病人對於美觀的改善，牙根敏感的消失都感到滿意。但此術式亦有較多的術後疼痛情形發生。整合性分析則發現皮下結締組織移植比起引導組織再生手術有較佳的牙根覆蓋率及增加較多的角化組織。

The use of subepithelial connective tissue graft (SCTG) for root coverage procedure had been carried out for over twenty years. However, its efficacy had not been systemically evaluated. The purpose of this paper was to review 1. clinical and patient-centered outcomes of root coverage surgery utilizing SCTG 2. harvesting techniques for SCTG 3. histology findings of SCTG 4. results of meta-analysis. The clinical outcomes included rate of root coverage, changes of probing depth, clinical attachment level and keratinized tissue. The patient centered outcomes included esthetics, root sensitivity, and postoperative morbidity. It was concluded that SCTG was able to accomplish root coverage, augment keratinized tissue, procure for patient satisfaction of esthetics and reduce root sensitivities. However, post-operative morbidity was also described. In addition, the data of meta-analysis of SCTG also showed that SCTG had better results in root coverage rate and keratinized tissue increasing than guided tissue regeneration method.
