摘要

臨床上常見牙齒因外傷斷裂、蛀牙或治療不當造成齒頸部缺損的情形。當缺損邊綠接近或與齒槽骨脊齊平甚至低於齒槽骨脊時，如果直接予以補綴物復形將會侵犯其生物性寬度，造成日後牙齦發炎腫脹及不美觀。針對這類斷裂牙齒，一般處理方法可分為拔除、蓄意再植、牙冠增長術、或強逼萌出術等。在保留牙齒之前提下，強逼萌出可減少犧牲齒槽骨，減少牙周囊袋及增加角化牙齦寬度，並且較不需改變本身及鄰牙的臨床牙冠長度，獲得較佳的美觀，然而，在進行強逼萌出術之前仍文須審慎考量病患齒之狀況，評估其預後並與患者達到充分溝通。本報告提出提出一利用簡單的矯正裝置治療正中門齒斷裂後贗復的病例作為類似病例治療時的參考。

In everyday practice, the general practitioner sometimes sees cases of fractured teeth or carious lesions around the cervical area. If we try to restore these subgingival or infraosseous dental lesions, we might impinge biological width to cause periodontal inflammation and unaesthetic result.
Possible therapeutic options include extraction, intentional replantation, surgical crown lengthening, and orthodontic forced eruption. Forced eruption provides undeniable benefits for the patients, such as minimizing alveolar bone scarification, and getting better crown root ratio than surgical crown lengthening. It can also reduce probing depth, increased the keratinized gingiva and maintained the healthy and esthetics. However, forced eruption is not appropriate in all cases, each teeth must be carefully analyzed before further treatment.
This is a case report of fractured upper right incisor, after orthodontic forced eruption, prosthesis was made. After 3 month follow-up, the result is still satisfied.
