自全民健保1995年開辦後,醫療機構為了因應衛生政策,無不導入資訊科技,以提昇其品質與效率,降低成本,進而創造組織競爭優勢。醫師為醫療服務之主要提供者,對於醫療機構而言,如何滿足醫師,尤為重要。而近來已導入大量之醫療資訊系統來協助醫師診療與決策,包含門診醫囑系統、住院醫囑系統、急診系統、醫療影像儲傳系統、醫療決策支援系統、電子病歷系統等。儘管資訊科技能帶來許多的優點,但過去研究顯示當資訊科技所提供的資訊過多、侵犯至私人生活等,以至於使用者無法負荷時,即會造成科技壓力,影響員工工作滿意度與生產力等負面影響。因此醫療機構在如此高度資訊化下,醫師不僅需熟悉各資訊系統之功能與介面,且需要在短暫之診療時間內透過資訊系統來完成其醫療行為,進而產生科技壓力。 本研究期望以Tarafdar, et al.(2011)提出之科技壓力模式來驗證醫療機構中醫師之科技壓力,與其對醫師心理之影響,並透過科技壓力抑制機制檢視醫師對醫療機構之因應策略是否能降低其心理影響。研究以調查法進行,結果可提供政策制定者在提升醫療機構資訊化的同時,應兼顧醫師之心理層面,降低其科技壓力,進而保障病患權益。
Since the Taiwan National Health Insurance (NHI) system was established in 1995, in order to response to health policy, health care institutions started to use all import information technology to enhance its quality and efficiency, reduce costs, and create organizational competitive advantage. Physicians are the primary providers of medical services for health care institutions in Taiwan, and therefore it is important to show concern for the psychological and emotional needs of doctors who are often very busy using outpatient systems, hospital medical advice systems, emergency systems, PACS, clinical decision support systems, and electronic medical records systems. Although information technology has many advantages, it can also result in technostress and impact employee job satisfaction, productivity and other issues. Therefore, in such a high degree of medical information technology in health care institutions, doctors will not only be familiar with the information system functions and interfaces, but technostress is also an important factor to study. In this study, we will use Tarafdar, et al. (2011) who proposed a model to validate the technostress on medical practitioners. In addition, we will see that the technostress inhibitors can reduce the psychological impact on doctors. We will use a survey method to study this issue. The results of this study can suggest to policy-makers of information technology when to make decisions of policy to health care institutions. They should also be more considerate of physicians’ psychological state of mind.
