Osteoma is a slow growing neoplasm and usually asymptomatic. Many patients noticed the lesion because of deformity of appearance. Osteoma can derive from endosteum, periosteum or developed within muscles. Histology characteristics are proliferation of compact and/or cancellous bone with surrounding connective tissue. Osteoma over head and neck region is uncommon. Extra-skeletal osteoma is extremely rare. We report a case of huge fibro-osseous lesion over inner side of left mandible extend to infratemporal fossa in a 24 years old male. He carne to our Hospital for help because of gradually left facial swelling without pain since he was 14 years old. Clinical examinations revealed facial asymmetry without mouth opening limitation and difficultly in mastication. No palpable lymph node enlargement was noted. After comprehensive survey of the mass, complete excision of the lesion was performed under general anesthesia. Post-operative pathology report revealed an extra-skeletal osteoma which is very rare in documentation.
