Rare Entity of Intraosseous Schwannoma in Mandible-A Case Report

Schwannoma, also called neurilemmoma, is a relatively uncommon and slow-growing benign tumor that arises from Schwann cells. Schwannomas in the head and neck regions primarily appear in soft tissues, and intraosseous cases are very rare. In this article, we present a case of intramandibular schwannoma in a 30-year-old male. A painless radiolucent lesion on the right side of mandible was discovered during a routine dental examination. Panoramic radiography and computed tomography revealed a well-defined unilocular ovoid radiolucent lesion along the right mandibular canal measuring approximately 1.4x1 cm. Clinical examination showed intact mucosa without bony expansion. The patient underwent mandible tumor intracapsular enucleation under general anesthesia. The histopathology and immunohistochemistry analysis confirmed the final diagnosis of mandibular schwannoma deriving from the inferior alveolar nerve. Regular follow-up showed no recurrence or complications.

許旺氏神經髓鞘瘤又稱作神經鞘瘤，是一種來自於許旺氏細胞，非常罕見且生長緩慢的良性腫瘤。在頭頸部區域，較常出現在軟組織內，骨內案例非常少見。本病例報告一名30歲男性，在常規牙科檢查中意外發現下顎骨右側內一射線透射的無痛病灶，環口放射線攝影及電腦斷層攝影影像上可見單個界線清楚、卵形且沿著下齒槽神經管走向的放射線可透之病灶，大小約1.4厘米X1厘米。臨床檢查口內黏膜完整且無骨性擴張。治療計畫為於全身麻醉下進行顎骨腫瘤囊內手術摘除。最終組織病理報告顯示為來自下齒槽神經的下顎骨許旺氏神經髓鞘瘤。病人於術後定期追蹤，無復發跡象及併發症。
