「住院病患對護理人員可信特質感受」量表編制

Establishing a questionnaire perceptions of the hospitalized patients toward their nurses possessing Trustworthiness

摘要


研究顯示,人際互動中,一個人具有某種特質讓對方感受可信,會讓對方願意與你合作。病患住院過程中,護理人員的特質讓病患感受到可信是成功互動關係的必要因素,而這關係影響病患能否遵行醫囑、執行持續性照護及健康促進行為,也是預測照護成效及滿意度評價的指標。本研究主要探討護理人員可信特質內涵,並經驗證分析,建構一份具有信、效度的「護理人員可信特質」量表。 “護理人員可信特質量表”經專家效度及217名住院病患預試,進行探索性因素分析,而後再以3家醫院308名內外科住院病患進行正式施測,以進行項目分析及驗證性因素分析(Confirmatory factor analysis,簡稱CFA)。確認量表包含「專業責任」、「護病溝通」及「個別性護理」三個構面,共有19個題項,具有良好的聚合效度、區別效度,以及建構信度,且模式適合度良好。期望將研究成果讓護理人員於病患住院過程中,可進行自我或同儕評估自己的特質與病患心目中的可信特質之差距,作為自我訓練與單位培訓人員的基石。

Studies have shown that a person having characteristics, which make people feel trustworthy would promote the willingness of others to cooperate in interpersonal activities. During hospitalization, the haracteristics of nursing staff, which makes patients feel the credibility towards them is a necessity of successful interaction. Such relationship could influence patients’ compliance of following doctor's orders, and ducting continuous care and health-promoting behaviors. It is also an indicator to predict the effectiveness of care and satisfaction.This study investigates the contents of nurses’ trustworthy characteristics. It builds a reliable and effective "nurse credible characteristics" scale based on validation and statistical analysis. "Nurse credible characteristics scale" was pre-tested for its expert validity and by 217 hospitalized patients for analysis of exploratory factors. Afterwards, the scale was handed out to 308 surgical or internal hospitalized patients in three different hospitals for formal test. Data was collected to carry out the item analysis and the confirmatory factor analysis, or referred to as the CFA.The scale is composed of three dimensions: "professional responsibility", "nurse-patient communication" and "individual care". It has a total of 19 items with good convergent validity, discriminant validity, construct reliability, and good model fit. The scale is expected to be used by nursing staff to self- or peer-evaluate their trustworthy characteristics. The outcome would be compared with how the hospitalized patients really feel about their nurses. The study results will serve as a cornerstone of self-training and unit personnel training.
